Client Date & Time Salon Use Only: Therapist Location I Acknowledge & represent that I have read the foregoing waiver of liability Agreement, understand it and sign it voluntarily as my own free act, no oral representations, statements, or inducements, apart from the foregoing written agreement, have been made. I am at least eighteen (18) years of age and fully competent. I execute the agreement fully intending to be bound by it. If signing this disclaimer for a person under the age of 18, the singee must prove that they are the parent or guardian. Identification will be required from both parties. The minimum age for any tinting or semi-permanent treatment is 16 years of age. Waiver of Liability Agreement Seema's Beauty Salon have advised me to undertake a patch test before receiving any eyebrow or lash tinting, extensions or lifting treatment. This is because the products used in such treatments can cause adverse reaction in some people. I understand that the eyebrow or lash treatment that I am receiving may cause an adverse reaction, owing to the interaction between the products used and particular types of skin and make-up. Contrary to the advice received, I have decided to receive the eyebrow/lash treatment/ wax/ without undertaking the patch test. I hereby RELEASE, WAIVE DISCHARGE, AND COVENANT NOT TO SUE Seema's Beauty Salon their officers, agents or employees from any and all liability, claims, demands, actions and causes of action what soever arising out of or related to any loss, damage or injury that may be sustained by me from the use off any products, whilst undergoing eyebrow or lash treatments by Seema's Beauty Salon. if there are any issues with the treatments all client s must consultee/inform/ must come as soon as possible to Seema's beauty salon (in emergencies situation/ out of hour clients can / may contact doctors / hospital) if required Proof id client & parent id
